Understanding Depression and Treatment Options

Depression—clinically referred to as major depressive disorder or clinical depression—is a serious mood disorder that affects how a person thinks, feels, and functions each day. It goes far beyond temporary sadness or emotional stress. Depression is persistent, often overwhelming, and can interfere with relationships, work, physical health, and overall quality of life. While the experience of depression varies from person to person, it is a legitimate medical condition that deserves thoughtful, professional care.

Research suggests that depression affects roughly 15%–20% of people at some point in life, and for many, symptoms can return, with recurrence rates estimated between 30%–40% [1]. These numbers highlight just how common—and complex—depression can be over time.

There are multiple forms of depression, some of which develop in response to life circumstances or seasonal changes. Major Depressive Disorder and Common Symptoms

Major depressive disorder (MDD) is one of the most common and severe forms of depression, affecting more than 8% of adults in the United States. To receive a diagnosis, an individual must experience five or more symptoms for at least two weeks, with symptoms significantly disrupting daily life.

Common symptoms of major depressive disorder include:

  • Persistent sadness or feelings of emptiness

  • Loss of interest or pleasure in once-enjoyed activities

  • Changes in appetite or weight

  • Sleep disturbances, including insomnia or oversleeping

  • Fatigue or low energy

  • Difficulty concentrating or making decisions

  • Feelings of guilt or worthlessness

  • Recurrent thoughts of death or suicide

Symptoms may vary in intensity and duration, and certain forms—such as seasonal depression—may follow predictable patterns.

Treating Depression Effectively

Depression is typically very treatable, especially when it is addressed early. Most treatment plans involve a combination of evidence-based psychotherapy—such as Cognitive Behavioral Therapy (CBT)—and medication when appropriate. Treatment-resistant depression (TRD) is typically defined as depression that does not improve after at least two antidepressant treatmentswith different mechanisms taken for an appropriate duration. Research suggests that roughly 30% of people receiving standard depression care may develop TRD, highlighting the need for additional treatment pathways.

For individuals with severe TRD, clinicians may consider more advanced interventions alongside ongoing therapy and medication management. These may include:

  • Electroconvulsive therapy (ECT): uses controlled electrical stimulation under medical supervision to reset disrupted mood-regulation circuits

  • Transcranial magnetic stimulation (TMS): uses magnetic pulses to stimulate targeted brain regions noninvasively

  • Vagus nerve stimulation (VNS): delivers electrical signals to the vagus nerve to influence mood-related brain pathways

  • Deep brain stimulation (DBS): involves surgically implanted electrodes that stimulate specific brain circuits linked to depression

Among these methods, deep brain stimulation (DBS) has drawn particular research interest.

DBS is considered one of the most promising—but also most invasive—experimental approaches for severe TRD. The procedure involves placing two thin probes into specific brain areas and connecting them to a small impulse generator implanted under the skin of the chest. This device sends carefully controlled electrical signals through the probes to influence brain circuits involved in mood regulation.

While the idea of using electricity in the brain can sound alarming, it’s important to understand that modern DBS bears little resemblance to outdated portrayals of “shock therapy” often seen in films. Today’s DBS techniques rely on precise brain imaging, individualized targeting, and carefully calibrated stimulation, making the process far more controlled and refined than early historical practices.

Emerging research is also exploring closed-loop DBS, a newer approach that adjusts stimulation in real time based on a person’s brain activity rather than delivering constant signals. This adaptive model may improve precision and reduce side effects, though it remains experimental and limited to research settings.

DBS relies heavily on stimulating the right neural pathways, not just a single brain location. Modern research uses advanced imaging to map each person’s unique brain wiring before treatment, helping doctors place stimulation more precisely.

Through this approach, clinicians aim to influence systems involved in:

  • Mood regulation

  • Motivation and reward

  • Stress response

  • Emotional threat processing

Across studies, approximately 50% of carefully selected patients experience meaningful improvement, though results vary based on the brain area targeted and how treatment is adjusted over time. Because DBS is still considered experimental and is not effective for everyone, limited large studies and the complexity of treatment-resistant depression make clear recommendations difficult.
