What Is EMDR Therapy and How Does It Work?

Eye Movement Desensitization and Reprocessing (EMDR) is an evidence-based psychotherapy designed to reduce psychological distress related to trauma. It is most commonly associated with the treatment of post-traumatic stress disorder (PTSD), but it is also used to address other trauma-related mental health conditions. EMDR is based on the idea that traumatic memories can become “stuck” when they are not processed, continuing to trigger emotional and physical reactions long after the event has passed.

When traumatic memories remain unprocessed, everyday sights, sounds, smells, or thoughts can cause the brain and body to react as if the trauma is happening again. This can lead to anxiety, emotional distress, intrusive thoughts, and other PTSD symptoms. EMDR therapy aims to reduce this distress by helping the brain reprocess traumatic memories so they no longer carry the same emotional intensity.

How EMDR Therapy Works

EMDR uses a structured, interactive approach guided by a trained therapist. During sessions, individuals briefly focus on a specific traumatic memory while engaging in bilateral stimulation—most commonly side-to-side eye movements. For those who cannot use eye movements, therapists may use alternating audio tones or rhythmic tapping instead.

This process occurs gradually and in a controlled environment. Over time, the brain can process the memory more effectively, allowing it to be stored in a way that no longer triggers intense emotional or physical reactions. The memory itself does not disappear, but it becomes less distressing and easier to recall without feeling overwhelmed.

Researchers believe EMDR works through several mechanisms. One leading explanation is the working memory theory, which suggests that focusing on a traumatic memory while performing eye movements reduces the brain’s ability to relive the memory with full emotional intensity. Another theory proposes that EMDR promotes physiological relaxation, helping shift the nervous system out of a fight-or-flight response and into a calmer state.

What to Expect During EMDR Treatment

EMDR therapy follows an eight-phase treatment model and typically takes place over multiple sessions, often around twelve, though timelines vary. Early sessions focus on assessment, preparation, and teaching coping skills such as mindfulness and grounding techniques. Later phases involve targeting specific memories, reducing distress, replacing negative beliefs, and monitoring emotional and physical responses. Progress is reviewed throughout treatment to ensure safety and effectiveness.
